Future-proof primary care

Research theme

The shift from intramural care towards primary care has begun, but there are other considerable challenges. Worldwide, the number of people with complex care needs is on the rise. Thresholds obstruct the access to healthcare that is still too supply-driven. An academic workplace, where we do practice-oriented research together with all healthcare actors, is a solid basis for further research and innovative projects. We do not only focus on disease and healing, but also on health and wellbeing by means of prevention. We describe care trajectories from the experiences of caregivers and care users. In this way, we focus on integrated, personalised care that is accessible to all.

Equal collaboration with caregivers, organisations, and the citizen

What are we researching and for whom?

The importance of future-proof primary care is no longer up for discussion. But there are many challenges. Worldwide, the number of people with complex care needs is on the rise. There is health inequality because of thresholds that obstruct the access to care. And the care is still too much supply-driven. We want to provide an answer to those challenges. On the terrain, and in organisations where we are working on local solutions by having education, research and practice collaborate. We do not only focus on disease and healing, but also on health and wellbeing by means of prevention. The healthcare is coordinated with the needs those people experience in their lives. We start from the experiences of caregivers and care users to describe care trajectories. Together we remove thresholds and expand solutions. Towards the policy, we take on the responsibility of notification.

Desired outcome

For a more integrated care and its implementation, we work on innovation with care and welfare professionals. We focus on integrated care with a tailored offer. In this way, we can reach everyone. An academic workplace, where we do practice-oriented research together with all healthcare actors, is a solid basis for further research and innovative projects.

Research lines

For more integrated care, we work with care professionals. We focus on care innovation, up to and including the implementation. We are starting from an academic workplace and want to strengthen the primary care by means of a clear focus on prevention, integrated care and neighbourhood care.

Academic workplace for strong primary care

Blended aan de slag met welzijn en geestelijke gezondheid

We start an academic workplace. That is a sustainable, structural partnership between Artevelde UAS (research and education) and final users, professionals, organisations, policy (the professional sector) with scientific knowledge development and innovation as a goal.

The strengthening and extension of integrated primary care

Samenwerken

We support counsellors and organisations in the implementation of the chronic care model. The result is purposive care, integral care, inclusive care, and better interprofessional collaboration. We also identify local patient streams and care processes.

More proactive health promotion and disease prevention

We are strengthening caregivers to engage in proactive and recreational prevention. We research how we can support we caregivers and care organisations in the adjustment, implementation, or development of new and existing methodologies and tools.

Impactful neighbour-oriented care

To organise tomorrow's care, the neighbourhood level is a vital element. That is where informal help and formal care can build on one another. Caring neighbourhoods are a story of customisation, starting from the wishes and talents of the neighbours. The research identifies good practices, both in a rural and urban context, and focusses on levers for sustainable social change.
